a faded oval seal together with an illegible ownership statement (1r); a square seal with a partially smudged Persian ownership statement under another erased statement (1r); an instruction for the preparation of medicine deriving from Kifāya-yi Manṣūrī (Manṣūr ibn Ilyās, 8-9th/14-15th c.) and the invocation yā kabīkaj on paper added in the restoration of the codex (1r); a librarian stamp of inspection dated 1328/1949 (1v and 87v); a hardly legible basmala (inner back cover).

A single clear nastaʿlīq hand in black; chapter titles rubricated, paragraph beginnings partially highlighted with red overlines; many final hamzas and shaddas, maddas mostly indicated and frequent use of lām-alif mukhaffafa. No tables or diagrams; mostly undotted abjad notation in red ink. Textual dividers inserted in red, especially in Book IV. Dimensions: 220×115 mm (Dānishpazhūh); 15 lines per page. Good condition, minor water damage on the last folios, some pages repaired with pasted paper slips; binding becoming loose, one folio after f. 1v lost [before the manuscript was foliated]; green leather cover over paper pasteboards, heavily damaged, decorated with a blind stamped medallion with pendants. Type III binding.

According to the preface, a translated version by Ḥunayn ibn Isḥāq, i.e. possibly the translation by Ibrāhīm ibn al-Ṣalt (see Ullman). Glosses by Thābit ibn Qurra to the first book, included in the main text. Minor marginal corrections in the hand of the scribe mostly indicated by sigla and ṣḥ. Some annotations in different hands.
